1. What did the guys want who were talking to the owner of the restaurant where Reacher was eating?

There were two guys at the bar talking with the owner of the restaurant. They wanted the owner to pay them $200 a week protection money. Then when business improved, they wanted $400.

2. When was Reacher afraid in cities and why did he change his mind?

Growing up, Reacher's mother had taught him to be afraid of cities because they were full of tough, scary guys. From his observations, he knew that his mother was right. Then suddenly, he realized that he was the scary guy and people were afraid of him. He looked at his reflection in a store window and saw that his size was intimidating and it frightened people. After that he knew that for every city person he needed to be afraid of there were 999 others who were a lot more scared of him.
