Why was Kino suspicious of the doctor's white powder?

He suspects that the doctor is giving his son something that is not a 'cure' but a slight poison to cause his son to become ill again. He believes that the 'doctor' is doing this in order to extort money from him and learn where he's hidden the pearl. As it turns out....he's right.
