Why is it odd that Hassan smiles more when he has the cleft palate than after it is fixed?

Typically, children, like Hassan who have a deformity which can be seen by others, are shy and backward, and would smile more after it is fixed. Therefore, it is odd that Hassan smiles more before his cleft pallet is corrected.
