On February 11, 1991, "pharmacy nurse Pam Allen had brought a suspicious IV bag to the desk of Saint Barnabas’s risk manager, Karen Seiden." The IV bag was sent to a pathology lab and it was revealed to contain insulin; three days later, patient Anna Byers almost died repeatedly from insulin overdose.
