People perceive Hyde as deformed because, well, he IS deformed. He has a way of walking that is hunched over, giving the impression of a hunched back. He drags a leg, seeming to suggest some deformity. His eyes are not focused, one is larger than the other. His facial features and teeth are not normal, either.
