What is the setting of Stone of Tears by Terry Goodkind?

The story takes place in a mythical land referred to as the Midlands. Physically the land looks very earth like, but many of the creatures that inhabit the land are linked with magic, such as dragons and gars, which are small creatures that look somewhat like small dragons. The author has also given the world of the dead a link to the world of the living, so some of the setting occurs in the land of the dead. Richard and Kahlan hold a ceremony to call their ancestors in which Darken Rahl escapes, and Denna comes to help Richard and Kahlan. Later in the story Denna comes once again and takes Richard and Kahlan to a peaceful place to talk that appears to be in the world of the dead.
