Sikander is the narrator's father. He immigrates to the United States in the late 60s from Pakistan. He initially comes to America to work as a cardiologist. He becomes enamored of the American way of life, and so he abandons his medical profession in order to go into real estate, a profession that he views as more American. After his properties fail to generate revenue, he returns to medical practice. As a leading cardiologist he is sought out by Donald Trump for treatment. The two become friends for the duration of Donald's treatment. During this time, Sikander learns to indulge in the pleasure-seeking thrills of American culture: drinking, gambling, and having sex with a prostitute.

His brief run as Donald's friend in the 90s causes Sikander to support Donald politically when he runs for president in 2016.

In 2017 Sikander is tried for malpractice as he succumbs to alcoholism and gambling addiction. After he loses all of his money to his gambling addiction he moves back to Pakistan in order to escape his creditors. Before he leaves, he attempts to introduce Ayad to his mistress, Caroline.
