It's a tragedy, so almost everyone dies. 

Polonius dies by a sword meant for someone else (wrong place, wrong time)

Laertes dies when he is poisoned with his own sword. 

Gertrude drinks poison. 

Ophelia drowns herself.

Hamlet's father dies on the battlefield.

Claudius dies by poison. 

Last, but not least, Hamlet dies by a poisoned sword.
