What is the setting of Arkham Asylum by Grant Morrison?

The graphic novel is set in Arkham Asylum, home to the madmen of Gotham City. The asylum is a fixed part of Batman's world, the home for the criminally insane where Batman's villains are sent, repressed from the world, and from which they periodically escape, much like repressed impulses erupting out in neurotic behavior.
