What is the setting of A Storm of Swords by George R.R. Martin?

The novel's setting is split between scenes in the North beyond the wall and Castle Black, to the Kingdoms inside the wall, most notably Riverrun and Kings Landing. Throughout the novel, Arya, Jaime, Brienne, and others pass through wild woods and swampy wetlands. The feel of the day and age is around the early 1100's.
