Dr. Spivey's heart is is in the right place but, like the other men, he is unable to stand up to Nurse Ratched when it counts. He supports the men in their various schemes for respite in the hospital but always backs down from nurse Ratched's wrath. In this way, he is harming the men. It is implied that Spivey is addicted to opiates which puts him in a compromising position with Nurse Ratched.
