It is set around 1960, when the book was written, which was a time of great pressure to conform.  McMurphy isn't really crazy when he arrives, but is labeled as sch because he is a non-conformist.  The ward for the psychologically disturb makes him into a patient, and then lobotomizes him, only because he is a threat to order.  In doing so, the mental health establishment of the time is skewered as nothing more than a tool of the rigid social order.
