What does the fog represent in One Flew Over the Cuckoo's Nest?

The 'fog' is a very important symbol and implied metaphor throughout the novel. It represents the 'fog' that the hospital keeps everyone in, in order to hide reality; keeping them all blind to the real world. The Chief imagines fog because the army used it to hide airstrips from enemies when he was in World War II. The drugs that they use to sedate the patients is also a type of fog.
