What is “Fifth Business” ? How do you account for the title?

Fifth business means the person always in the background of life facilitating other while he or she remains in the shadows never really living. As described by Liesl late in the novel, Dunstan is "Fifth Business," the type of person always destined to play a supporting (and never a lead) role in life.
