Ursula Ruth Strumpfler Gallivan, 81, a charter member and visiting chairman of Mended Hearts Association, a support group for people who have had heart disease or medical procedures, died of congestive heart failure Dec. 4 at a daughter's home in Clemmons, N.C. Mrs. Gallivan, a former registered nurse, joined Mended Hearts after undergoing surgery to repair a heart valve problem at Walter Reed Army Medical Center in 1956. At the time, her husband, John D.
