The leading causes of death and disability during the past century have shifted from infectious diseases to chronic conditions that have a variety of behavioral, social, and psychological underpinnings. It is therefore appropriate at the dawning of the new millennium to attempt to integrate and synthesize the rich social science literature on health, to transcend the gap between research and health practice, and to direct future research.
